FEES

Insurance

We are out-of-network for all insurance plans. As an out-of-network provider we provide you with a “superbill” to submit to your insurer for possible reimbursement. Please contact your insurance company for details on your level of possible reimbursement for out-of-network visits. The initial visit code is 99205 and the follow-up visit code is 99215. The exception is for Medicare, for which we do not offer a “superbill.”

Initial Visit Costs

All payment is due at the time of service.

SPINE SERVICES
Initial assessment visit for low back pain: $2,350
This McGill Method three-hour initial consultation:

  • In – depth  medical interview
  • Thorough physical examination
  • Review of original spine imaging
  • Coaching on posture, movement, and exercise
  • Detailed report

CHRONIC INFLAMMATORY RESPONSE SYNDROME SERVICES 

Initial assessment visit for Chronic Inflammatory Response Syndrome: $1,250

This one-hour initial consultation includes:

  • History and physical examination
  • Environmental report review
  • Visual contrast sensitivity testing
  • Mental Objective Cognitive Assessment
  • Nasal Swab Obtainment (Lab Fee Separate)
  • In-Clinic Spirometry Testing  

Scheduling

We do not double-book patients, so it is important that you give us adequate notice if you need to cancel or change an appointment. For initial consultations, a non-refundable booking fee of $250 is required for an initial one-hour appointment and of $350 for an initial three-hour appointment. Appointments are made on the scheduling line at 626.838.5485.

Additional Services

Certain additional services may incur out-of-pocket charges. Some examples are:

  • Time spent by your doctor filling out legal forms (e.g., disability or FMLA forms); however, please note that our doctors will not complete such forms until you have been an existing patient with an established physician-patient relationship for at least 3 months
  • Prior authorizations for medications, labs, and imaging
  • Prolonged clinical phone calls or emails between visits
  • Physician legal expert services

Forms of Payment

We accept cash, check, debit card, and all major credit cards. Your card number will be held on file for ease of payment in the future. You can also pay with your Health Savings Account debit card.
